Building on the success of our 2008-13 Big Lottery Fund Wellbeing Programme and our innovative work on the Right Conversation, Right Time project, we are delighted to have secured further lottery funding to enable us to develop our Champion model and build partnerships with citizens and services in new locations.
Over a two year period we will be working closely with citizens, Clinical Commissioning Groups, Mental Health Trusts, GP Practices, local authorities and the voluntary sector to develop new relationships and conversations, creating the conditions within which they can co-produce solutions that address the health and wellbeing needs of their community, in order to:
- Achieve better outcomes
- Transform relationships
- Create capacity
- Find different ways to meet increasing demand within limited resources
Working in Barnsley, Bradford, Calderdale, East Riding, Sheffield, Shropshire and the North East, the programme will engage and train local people to become:
- Practice Health Champions in GP practices
- Youth Health Champions
- Pregnancy & Early Parenthood Champions
